Comments:
An unannounced monitoring inspection was conducted on June 7, 2017. There were 21 children present upon arrival to the center with four staff members. Additional staff were on site for administrative and support purposes. There is a total enrollment of 49 children. The children were observed having lunch, during circle and center time, coloring, and handwashing. The infants were observed having tummy time, crawling and feeding in the Infant room. Three children's records were reviewed. Three staff records were reviewed. Medications are currently not being administered. The exit interview was conducted with the Program Director.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, in one of three children's records reviewed, the licensee did not ensure that each child shall have a physical examination before the child's attendance or within one month after attendance. Evidence: During the inspection conducted on June 7, 2017, there was no documentation of a physical examination in the record for Child #2 (child started January 4, 2017).

Plan of Correction: Make sure the physical is brought in by the parent.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the licensee did not ensure that each child's record was maintained with all required information. Evidence: 1. During the inspection conducted on June 7, 2017, in the record for Child #2 there was no documentation of the one parent's information to include the place of employment and work phone number. 2. In the record for Child #3, there was no documentation of one parent's place of employment and work phone number.

Plan of Correction: Registration forms will be updated today.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on June 7, 2017, the following was observed in an unlocked bathroom in the Infant room: bleach, disinfectant spray, air freshener and bleach/water solution. All items were labeled keep out of reach of children and caution or danger.

Plan of Correction: Make sure the door remains locked at all times.

Standard #: 22VAC40-185-440-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that cribs shall be identified for use by a specific child. Evidence: During the inspection conducted on June 7, 2017, an infant was observed in a crib with another child's name on it.

Plan of Correction: This was corrected during the inspection. The child no longer attends the center. I will make sure that each crib is properly labeled.

Standard #: 22VAC40-185-440-J
Description: Based on observation and inspection of the facility, the licensee did not ensure that there was at least thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. Evidence: During the inspection conducted on June 7, 2017, two infant chairs and a bouncy seat were positioned immediately in front of the access side of an occupied crib.

Plan of Correction: The items were removed during the inspection. I will make sure that the teachers are aware that the access sides should not be restricted or blocked.

Standard #: 22VAC40-185-500-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that a nonabsorbent surface for diapering or changing shall be used and that the diapering surface shall be used only for diapering or cleaning children. Evidence: During the inspection conducted on June 7, 2017, there was a small tear on the diapering pad in the Infant room and a roll of paper towels was on the changing pad in the Girls restroom.

Plan of Correction: The paper towels were removed during the inspection. The changing pad will be replaced with a new one.

Standard #: 22VAC40-185-550-C
Description: Based on observation and inspection of the facility, the licensee did not ensure that emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: During the inspection conducted on June 7, 2017, evacuation procedures were not posted in the center.

Plan of Correction: Make sure the evacuation procedures are posted.

Standard #: 22VAC40-185-550-D
Description: Based on observation and inspection of the facility, the licensee did not ensure that the center shall implement a monthly practice evacuation drill and a a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: During the inspection conducted on June 7, 2017, there was no documentation of an evacuation drill practiced in October and no documentation of two shelter-in-place drills practiced in 2016.

Plan of Correction: Make sure the fire drills and shelter-in-place drills are done and documented as required.

Standard #: 22VAC40-185-570-A
Description: Based on observation and inspection of the facility, the licensee did not ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Evidence: During the inspection conducted on June 7, 2017, two infants were observed in high chairs without the protective belts fastened.

Plan of Correction: Make sure that infants are fastened in at all times.

Standard #: 63.2-1720-F
Description: Based on observation and inspection of the facility, the licensee did not ensure that no employee shall be permitted to work in a position that involves direct contact with a person or child receiving services until an original criminal record clearance or original criminal history record has been received, unless such person works under the direct supervision of another employee for whom a background check has been completed. Evidence: During the inspection conducted on June 7, 2017, Staff #1 (start date 6/1/17) was observed working alone in the Infant room. Staff #2 was observed working alone at times in the Toddler room. Both staff persons, during the time of working alone, were not within direct supervision of another employee for whom a background check had been completed.

Plan of Correction: Staff members will be supervised until the background check results come back.
