Please be sure to audit your medications on site to ensure that medications that are no longer needed are either returned or disposed of.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 43 children with 3 staff present during the inspection. Children were observed having snack which consisted of banana bread and milk. Nice transition from snack time, to hand washing to both small and large group activities. Some children observed watching a Disney movie, playing video games, completing a table top bead activity and individual reading. Some violations found today. Three children who have medications with authorizations were reviewed. MAT (medication administration training) training and first aid/CPR certifications were checked today. Staff records are reviewed annually in the administrative office. This inspection began at 2:40pm and ended at 5:40pm. Standard #: 22VAC40-185-60-A
Description: Based on review of child records, one child's proof of identity document was incomplete. Evidence: Child #2's proof of identity did not include the reviewer's name who verified the proof of identity document

Plan of Correction: We will ensure that the main office documents the reviewer's name that viewed the original documents for proof of identity

Standard #: 22VAC40-185-60-A-7
Description: Based on child record review, actions to be taken in an emergency was incomplete for one child's food allergy. Evidence: Child #2 has a peanut and sesame allergy. Child #2's health exam on file indicates the need for an individual health care plan needed for the use of an EpiPen. The physician recommended plan was not found in the child's record.

Plan of Correction: Center will clarify the need for this plan for this child.

Standard #: 22VAC40-185-240-D-5
Description: Based on review of staff records, no documentation of daily health observation training was found. Evidence: A review of all staff records who were present during inspection was made. No documentation that any staff present has current daily health observation training.

Plan of Correction: All staff have had the training, supervisor requested copies of the training. Supervisor will obtain copies from the main office tomorrow.

Standard #: 22VAC40-185-510-N
Description: Based on review of all medications, one medication was not disposed of by the center. Evidence: Visine eye drops for Child #6 who no longer attends the program was not returned to the parent or disposed of.

Plan of Correction: Center will dispose of this medication today.

Standard #: 22VAC40-185-560-G
Description: Based on observation of snack time, food brought from home was not labeled. Evidence: Two children were observed eating snack brought from home, both snack containers were not labeled and dated.

Plan of Correction: Center will ensure that all food brought from home is labeled and dated.
