You are reminded that one lock down drill is now required on an annual basis.

An unannounced Subsidy inspection was conducted this date. 43 children present with a total of 3 staff present. CPR and First Aid certifications verified for all 3 staff. Pre-Service training completed and certificates on file. A review of the emergency preparedness plan was made and re-unification and lock down procedures were included in the Center's plan. Three children diagnosed with food allergies. One violation found regarding food allergies and one violation found regarding children's records. Thank you for your cooperation today. Standard #: VENDSUB-000-033-F
Description: Based on child record review and staff interviews, staff are not aware of whether one child has a diagnosed food allergy. Evidence: Child #3's health exam dated 1/17/14 documents a food allergy to eggs. Staff were unaware of whether the child has a diagnosed food allergy. LI observed Child #3 eating banana bread provided as snack. LI reviewed the packaging for the banana bread and eggs were listed as an ingredient.

Plan of Correction: This child does not react to food with eggs in it. Center will clarify the severity of the egg allergy with parent and obtain physician documentation that the allergy no longer exists.

Description: Based on child record review, children's records were not complete. Evidence: The model form used for the Parks and Rec program does not include work addresses for parents.

Plan of Correction: The main office will revise this form to include work addresses for parents.

Standard #: VENDSUB-000-006-B-11
Description: Based on child record review, allergy care plans were not on file for children with diagnosed allergies. Evidence: Child #2 has a diagnosed allergy to peanuts and sesame. Child #4 has a diagnosed allergy to eggs. Allergy care plans were not on file for these two children.

Plan of Correction: Main office will clarify the need for an allergy plan for both of these children.
