Comments:
An unannounced monitoring visit was conducted today from 9:50-11:50. All rooms were in ratio with direct care staff. Children were observed in free play, room choices, infant care, reading, circle time and transition to lunch, Lunch served today was Turk-a-roni, spinach salad, fresh fruit and milk. Infant care was observed and diapering procedures were followed. Bottles were correctly labeled and infants were observed sleeping on their backs in occupied cribs. One newly hired staff record and medication authorizations were reviewed. Emergency medication is on-site with six staff having Medication Administration Training (MAT) and two having the MAT Diabetic module. Evacuation drills were up to date. Standard #: 22VAC40-185-40-E
Description: Based on observation, the center is not following its own policy to ensure that medication is stored in the teacher's locked closet. Evidence: Two medications were stored in the emergency backpack which was hanging from a hook in the pre-k classroom.

Plan of Correction: Teachers have been notified and retrained on medication storage, handling and usage.

Standard #: 22VAC40-185-240-A
Description: Based on observation, two classroom teachers were unable to demonstrate a knowledge of their job responsibilities when they did not know where to locate emergency medication in each of the classrooms. Evidence: When the Licensing Inspector walked through the two classes, both teachers knew that a child had emergency medication but were unable to locate the medication. Teacher #1 found the medication when the Inspector suggested she look in the emergency bag. Teacher #2 did not find the medication- it was found later by the center management in the classroom closet.

Standard #: 22VAC40-185-260-A
Description: Based on observation, the annual fire permit is expired. Evidence: The posted fire permit expired 5/31/2017.

Plan of Correction: Center management has reached out to the Fire Marshall and made plans to pick up the new permit as it had not been mailed to the correct address. Posted 6/5/2017

Standard #: 22VAC40-185-280-B
Description: Based on observation, a bag of potting soil which states "keep out of reach of children" was not stored in a locked place. Evidence: The bag of soil was sitting outside the door of classroom Two's-1 and accessible to children.

Plan of Correction: Soil has been relocated to locked storage closet.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, the center is not following procedures to ensure long term authorizations are current. Evidence: Written authorizations signed by the parent and physician expired July 2016 for child #2's emergency medication. Based on medication review, a medication was outdated. Evidence: A medication for child #1 expired 5/2017.

Plan of Correction: Parents were immediately contacted and asked to provide updated medical documentation and medicines as soon as possible.
