Comments:
An unannounced non-mandated SHSI inspection was conducted today. There were 26 children in care aged 5 months-8 years. There were 3 violations noted during this inspection. Thank you for your time and coo[eration. If you need further assistance please let me know. This inspection began at 9:15am and concluded at 1:15pm, conducted along with the renewal inspection.

Violations:
Standard #: VENDSUB-000-012-C
Description: Based on discussions with administrative staff the facility failed to maintain compliance with this standard as required relating to staff having CPR/FA. Evidence: No staff have completed the required 10 hours pre-service as of this inspection. Pre-service was to be completed by April 1, 2017 for the staff currently employed by the facility, as no new staff have been hired.

Plan of Correction: Staff will continue to work on completion of this training and it will be completed by the end of this month for all staff.

Standard #: VENDSUB-000-012-E
Description: Based on discussions with administrative staff the facility failed to maintain compliance with this standard as required relating to the vendor having CPR/FA. Evidence: The vendor did not have current CPR/FA as required. The document on record had expired as of March 2016.

Plan of Correction: The vendor and staff will be set up immediately for CPR/FA training, and renewed as required there after.

Standard #: VENDSUB-000-012-F
Description: Based on discussions with administrative staff the facility failed to maintain compliance with this standard as required relating to staff CPR/FA. Evidence: There are currently only 2 staff who have current CPR/FA. All staff were to have been certified in CPR/FA by April 1, 2017, as there have been no new staff hired.

Plan of Correction: The vendor and staff will be set up immediately for CPR/FA training, and renewed as required there after.
