Thank you for your time and assistance with the unannounced renewal inspection conducted today, June 13, 2017. The on-site inspection was from 9:45 a.m. to 4:10 p.m. On this date, at the time of arrival, there were 79 children in the care of 13 staff members. There were 109 children enrolled in the program. The records of four current staff members, twelve new staff members, and eight children were reviewed. The medication and medication paperwork were reviewed for six children. There were eight staff members and four children interviewed. Six classrooms and two playgrounds were observed. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. Lunch consisted of pork barbecue sandwiches, squash blend, black beans, pineapple slices, and milk. Some items observed and reviewed included: an art activity of constructing water slides, outdoor play, water play, choice time, infant feeding, diaper changing procedures, circle time, story time, an intergenerational activity, hand washing and bathroom procedures, lunch, nap time, dismissal procedures, required posted information, annual fire and health inspections, documentation of evacuation and shelter-in-place drills, injury reports, sunscreens, parental authorizations for administering sunscreen, first aid kit and emergency supplies. Standard #: 22VAC40-191-40-D-1-A Description: Based on review of records and verification from the director, the center failed to obtain background checks on the corporation's new board officers within the required time frame. Evidence: 1. The background checks for Board Officer A were not completed within 30 days of taking office. Board Officer A took office on January 1, 2017. The criminal history record check was obtained 05/17/2017. The search of the central registry had been sent off but had not yet been returned. The sworn statement was completed on 05/11/2017. 2. The background checks for Board Officer B were not completed within 30 days of taking office. Board Officer B took office on January 1, 2017. The criminal history record check was obtained 05/17/2017. The search of the central registry had been sent off but had not yet been returned. The sworn statement was completed on 05/11/2017. 3. The background checks for Board Officer C were not completed within 30 days of taking office. Board Officer C took office on January 1, 2017. The criminal history record check was obtained 06/02/2017. The search of the central registry was obtained on 06/06/2017. The sworn statement was completed on 05/25/2017. 4. The background checks for Board Officer D were not completed within 30 days of taking office. Board Officer D took office on January 1, 2017. The criminal history record check was obtained 06/05/2017. The search of the central registry was obtained on 06/06/2017. The sworn statement was completed on 05/11/2017. Plan of Correction: The executive director called to check on the two central registry searches that had not yet been returned. They were completed on 06/09/2017 and were sent back to the center on 06/12/2017. Looking forward, the executive director will complete all background checks on all new board officers after they are voted in at the November board meeting so that the checks will be completed within 30 days of the date they take office on January 1st of each year.
