An unannounced Subsidy Health and Safety Inspection was conducted on June 12, 2017 from approximately 7:50 a.m. until 10:00 a.m. Upon entrance there were 24 children and 4 staff present with children. The record review sample consisted of 6 children's records and 4 staff records. Children were observed playing with cars, trucks and coloring. Standard #: VENDSUB-000-006-B-11
Description: Based on observation, record reviews, and staff interviews , the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on June 12, 2017, there was one child (child #4) who was identified has having a food allergy (nuts) and there is not documentation of physician's information on how to address an allergic reaction.

Plan of Correction: The center supervisor will enaure that a written allergy care plan is in place for all children with diagnosed food allergies.

Description: Based on observation and vendor interviews, the vendor did not ensure within the first 90 days of employment or subsidy vendor approval that all staff have completed CPR and first aid training as required by this section within 90 days of the effective date of this agreement. Evidence: During the inspection on June 12, 2017, it was determined 2 of the 4 staff (staff #1 and staff #4) who were required to obtain CPR and first aid training did not have the certifications/training. Staff #1 date if hire was 2-17-2016 and staff #4 date of hire was in May 2016 as verified from the center.

Plan of Correction: Newport News PR&T maintains CPR & first aid certifications of all full time staff. Administration will continue to work toward certification of all other staff.

Standard #: VENDSUB-000-031-B-1
Description: Based on documentation review and vendor interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child when there is an evacuation, shelter-in-place or lock down. Evidence: During the inspection on June 12, 2017 the center director verified with the licensing inspector the emergency plan did not include any procedures for reunification.

Plan of Correction: The center supervisor will ensure that reunification plans are included in the emergency evacuation plans.
