Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-185-(4)-260-A (Annual fire inspection); 22VAC40-185-(4)-330 (Play areas);

Comments:
An unannounced monitoring inspection was conducted on 6/16/17 from 7:55am - 9:05am. During the inspection there were 11 children ages three months to five years old in care with four staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms. Records were reviewed for three children and six staff. There is no medication at the facility. Emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-350-E-2
Description: Based on observation, it was determined that the facility did not ensure that they maintain the required ratio of one staff for every five children whenever children 16 months old to two years old are in care. Evidence: 1. The Licensing Inspector entered the facility and observed staff #1 (Program Director) in the classroom adjacent to the kitchen with nine children. 2. When staff #1 asked how old the youngest child was, staff #1 stated 18 months.

Plan of Correction: The facility responded: Corrected during the inspection. We will ensure that we maintain the required staff-to-child ratios.

Standard #: 22VAC40-185-500-B
Description: Based on observation, it was determined that the facility did not ensure that a nonabsorbent surface for diapering or changing is used. Evidence: 1. The changing pad on the diaper changing table in the Infant classroom had a one inch tear on it. 2. Staff #2 (Infant teacher) confirmed that there was a tears on the diaper changing pad.

Plan of Correction: The facility responded: A new diaper changing pad will be purchased for the Infant classroom. Whenever a diaper changing pad is torn, it will be replaced.

Standard #: 22VAC40-185-550-D
Description: Based on a review of documentation and interview, it was determined that the facility did not ensure that a monthly practice evacuation drill for the most likely to occur scenarios was conducted. Evidence: 1. The emergency drill log did not have documentation that a practice evacuation drill was completed for May 2017. 2. Staff #1 (Program director) confirmed that a practice evacuation drill was not completed for May 2017.

Plan of Correction: The facility responded: We will ensure that a practice evacuation drill is completed each month.
