Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on June 19, 2017 from approximately 11:15 a.m. until 4:45 p.m. Upon entrance there were 56 children and 8 staff present with children. The record review sample consisted of 4 children's records and 7 staff records. Children were observed eating lunch and playing outside. Violations:
Standard #: VENDSUB-000-031-B-1
Description: Based on observation, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an evacuation or lock-down procedures. Evidence: During the inspection on June 19, 2017, there were not reunification plans or lock-down procedures available for review.

Plan of Correction: The program director will work with his supervisor to obtain new plan.
