Technical Assistance:
Staff under the age of 18 must be under the sight and sound supervision of an adult staff member. VA Preservice training can be counted as annual training one time. Please notify your inspector of suspension of services for more than 24 hours as a result of an emergency. Please notify your inspector ?preferably in writing ? of any serious injuries sustained at the facility - please see definition of serious injuries on pg. 3 of vendor agreement addendum.

Comments:
An unannounced subsidy health and safety inspection (SHSI) was conducted with the center director and center staff. A licensing inspection was also conducted on this date (see that report for details). There are currently 15 staff employed by the facility. Most staff have documentation of the mandatory VA Preservice Training and most are currently certified in first aid and CPR . There is one child diagnosed food allergies currently enrolled. The emergency preparedness plan will be reviewed for reunification plans and lockdown procedures. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required one a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. Standard #: VENDSUB-000-012-B
Description: Based on review of 8 staff files, the facility failed to have Virginia Preservice Training for all direct care staff within 90 days of employment. Evidence: Staff B (12/15/2016), Staff E (date of hire 3/6/2017), Staff G (date of hire 2/10/2017), and Staff H (date of hire 11/29/2016) did not have documentation of having completed the Virginia Preservice Training.

Plan of Correction: Staff will complete the training and will provide documentation to the center.

Standard #: VENDSUB-000-012-E-2
Description: Based on review of eight staff files, the facility failed to have first aid and CPR certification for all direct care staff within 30 days of employment. Evidence: Staff E (date of hire 3/6/2017) and Staff G (date of hire 2/10/2017), did not have documentation of having completed the first aid Training.

Plan of Correction: Employees will get first aid training in the next month.
