Comments:
An investigation was conducted to investigate a self-reported incident regarding supervision, care, protection and guidance and policies and procedures from 11:15 a.m. to 1:15 p.m. Staff interviews were conducted. Records and documentation were reviewed. There were no violations regarding the self-reported incident. There was a violation unrelated to the investigation. Standard #: 22VAC40-185-340-A
Description: Based on observation, staff failed to ensure the care, protection and guidance of children on the playground. Evidence: 1) On April 13, 2017, three children were observed playing in and around a composite play structure that had been marked off-limits by yellow caution tape because the owner stated she had determined that the resilient surfacing was insufficient. 2) For several minutes, one child, approximately three years old, was observed climbing on the composite play structure and wrapping himself in the yellow caution tape. 3) Another child, three or four years of age, was observed playing under the same play structure. 4) An older school- aged- child was seen tearing the yellow caution tape off the equipment. 5) Staff were observed to be watching the three children?s behaviors; No staff intervened or redirected the child from the off limits composite play structure area.
