Comments:
An unannounced monitoring inspection was conducted from 9:30 a.m.-12:45 p.m. with the center director and assistant director. 81 children, ages 5 months through 11 years old, were under the care and supervision of 13 staff. Child were observed having water play on the playground, playing Monopoly, putting together a puzzle, listening to a readaloud, and eating lunch. Lunch served today was fish sticks, whole wheat bread, corn, apple slices, and milk. Required information was posted. Both the fire and health inspections are current. The first aid kit is complete. The last emergency escape drill was 6/30/17. The last shelter-in-place drill was 4/5/17. One medication, five staff records, and seven children's records were all reviewed. A subsidy health and safety inspection (SHSI) was also conducted on this date (see separate report dated 7/7/17). Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of five staff records, the center did not have documentation of a tuberculosis (TB) test or screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: Staff B?s (hired 10/28/16) TB test was dated 12/1/16.

Plan of Correction: For future new hires, we will make sure that TB tests are done in the proper time frame.

Standard #: 22VAC40-185-240-A
Description: Based on review of five staff records, the center did not have documentation that staff received orientation training that includes job responsibilities, policies/procedures, playground safety procedures, child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibilities. Evidence: Staff A's (hired 8/30/16) record did not have documentation of orientation training.

Plan of Correction: The director conducted orientation training with Staff A during the inspection. Documentation of this orientation training was place in Staff A's record.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff records, the center did not have an original central registry report within 30 days of employment of a staff member. Evidence: Staff A's (hired 8/30/16) central registry report was dated 10/7/16. Staff B's (hired 10/28/16) central registry report was dated 12/29/16.

Plan of Correction: In the future with new hires, background check requests will be submitted on the staff's first day of employment.
