An investigation was conducted on June 22, 26, and 27, 2017 to investigate a self-reported incident regarding supervision of children: care and protection. An onsite investigation was conducted on June 27, 2017 from 9:15 a.m.-11:00 a.m. An exit meeting was conducted with the director on July 10, 2017. Based on observations, interviews, and documentation review, there was evidence to substantiate the incident. Description: Based on interviews and documentation review, staff did not ensure the care and protection of infants in care. Evidence: During the morning of June 21, 2017, Staff A stated that while fixing the label for Child A's bottle of breast milk she, "in error," labeled the bottle with Child B's name. At approximately 3:10 p.m., Staff B fed Child B that mislabeled bottle of breast milk. Staff were unaware that Child B was fed the wrong bottle of breast milk until the parent of Child B indicated at pick-up that the empty bottle did not belong to Child B.

Plan of Correction: The two teachers involved in the incident in the infant room will be retrained in safe bottle feeding immediately. (This was completed on 6/26/2017 for Staff A and Staff B. Signed off and in employee file.) Bottles will be better organized in the fridge, and will be checked when teacher transitions occur. (This was put into place immediately.) Bottles that are mislabeled at drop off will be corrected by the parent. If a mislabeling issue is missed at drop off, it will still be corrected by the teacher and checked by the co-teacher and MOD. (This was put into place immediately.)
