Comments:
An unannounced Health/Safety and Subsidy inspection was conducted on 7/11/17 at 10:00 a. m. and concluded at 1:30 p. m.. The inspector met with staff and standards were reviewed. The following staff-to-child ratios and activities were observed. Two-Four Years: There were eight children with one staff. The children were sitting at the table listening to a book on tape. The children later had their lunch which consisted of Spaghetti, Salad, Oranges and Milk. Infants: There were two infants with two staff. The staff and infants were playing on the mat on the floor. Five children records were reviewed. Four staff records were reviewed. No medications on site, including emergency medications. Conversations were held with several staff and children. Classroom observations were conducted in two classrooms.

Standard #: VENDSUB-000-005-B
Description: Based on review of the written record of daily attendance the center did not have all requirements available. Evidence: On 7/11/17, the daily attendance form that is currently being used did not have an arrival time and a departure time.

Plan of Correction: New forms will be created today.

Standard #: VENDSUB-000-006-A
Description: Based on review of children records the center did not have all required information on file. Evidence: On 7/11/17, child #1 and #2 did not have written allergy plans on file.

Plan of Correction: Will confirm with parents of allergy and request allergy plan from physician.

Standard #: VENDSUB-000-031-A
Description: Based on review of the written emergency plan the center did not have all requirements. Evidence: On 7/11/17, the emergency plan did not have lock down procedures specific to the center. Also, there were no detailed procedures regarding reunification.

Plan of Correction: Will update for detailed procedures for emergency plans.
