- Technical Assistance:
Technical assistance today included record keeping and logging of incident/accident reports.
An unannounced renewal inspection took place today. There were six classes present with 49 children and 8 direct staff. I observed children involved in dramatic play; print-making; easel painting; playing with toy cars and eating lunch. The outdoor areas were not inspected today due to monsoon conditions. Standard #: 22VAC40-185-40-J Description: During a review of records today, there were no written updated injury prevention procedures based on an annual or more documentation of injuries and incidents as required. Evidence: There was no available log to document incidents dating the prevention procedures today. Plan of Correction: We will start a 6 month review. It will be completed as soon as possible.
Standard #: 22VAC40-185-60-A Description: During a review of children's records today, some required items were not present Evidence: Of five records reviewed, two did not have addresses of emergency contacts; and five did not have information regarding previous child care; concurrent programs while enrolled and staff verification of proof of identification as required. Plan of Correction: We will add the missing information as soon as possible.
Standard #: 22VAC40-185-90--A Description: During a review of five children's records, the written agreement present did not include an acknowledgment that a parent would notify the center if any household member developed a contagious disease within 24 hours as required. Evidence: There was no written agreement in any of the five files reviewed today. Plan of Correction: The form was printed and we will have parents sign by 7/14, and replace the old form with the new form.
Standard #: 22VAC40-185-270-A Description: During today's inspection, some areas were observed to not be maintained in a safe/operable condition. Evidence: The two dorm -style refrigerators holding infant bottles and breast milk in the infant room were observed to be filled with a significant amount of frost. A large roll of brown craft paper was standing upright in an area used by children as they walked from the school-age classroom to the bathroom. Plan of Correction: Refrigerator: It will be done as soon as possible. The paper roll was removed during the inspection.
Standard #: 22VAC40-185-340-A Description: During today's inspection, a staff supervising a classroom was not able to ensure the care, protection and guidance of a child who exited the bathroom. Evidence: Child #1 was observed today crouching behind a bookshelf in an area outside the bathroom when the inspector and assistant director opened the door to the extended part of the classroom. Child # 1 was hiding and in the process of unrolling and re-rolling toilet paper. Staff #1 was observed to be sitting at the table, working with other children in the classroom. Child # 1 could not be seen by staff from the seat due to the partial wall. Plan of Correction: Due to the structure of the classroom, I will communicate with the teachers about supervision when children are in the restroom, or have them call for assistance when children are in the restroom.
