Comments:
An unannounced renewal inspection was completed on July 14, 2017. The purpose of this visit was to measure the facility?s compliance with most, but not all, of the licensing standards. The following ratios were observed: Infants and Toddlers-2:7; Twos-1:5; Threes-2:10;Fours/Fives-2:14;School age-2:20 The children throughout the facility were engaged in activity. Children in the toddler through four year old classes were observed playing outside, listening to stories, playing with toys, working in learning centers and working with manipulatives. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with manipulatives and supplies and items were available to the children. The following information was reviewed: fire and shelter-in-place drills, liability insurance policy, parent agreements, and staff training information. Medication is administered; medication authorizations were reviewed. Staff is certified in cpr/first aid, daily health screening and MAT. Last documented health inspection: 1/4/17 Last documented fire inspection: 12/13/16 During the inspection, five children's records and five staff records were reviewed. Approximate time on site: 9:59am to 12:30pm

Violations:
Standard #: 22VAC40-185-150-B
Description: Based on a review of a sample of children's records, the facility did not ensure that a child's physical examination report included all required components. EVIDENCE: The health report submitted for child #1, start date 6/30/16, was not signed by a physician, his designee, or an official of a local health department.

Plan of Correction: I will have the child's parent get a new signed physical examination form from the child's physician.

Standard #: 22VAC40-185-240-D-5
Description: Based on observations and documentation at the facility, the facility did not ensure that there was at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. EVIDENCE: There are two staff members that have training in daily health observation. At the onset of the inspection, neither of the two staff with the training was present at the facility. A staff person who had obtained the daily health observation training arrived later during the inspection.

Plan of Correction: We will get more staff trained in daily health observation training to ensure that someone with the training is always present at the facility. Until then, we will ensure that at least one of the staff trained will always be on site.
