An unannounced monitoring inspection was conducted on July 17, 2017. The site director was present. There were 62 campers with 6 staff. Campers were divided into 4 groups and were engaged in camp activities. Evacuation drills had been conducted. Four staff files and 6 children's files were reviewed. Inspection began: 9:50 Inspection ended: 12:30

Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on a review of 6 children's records, the center did not have a written allergy care plan for each child with a diagnosed food allergy, that included instruction from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child #2, Child # 5, and Child # 6 had a food allergy listed in their enrollment information. There were no allergy care plans on file for the three children.

Plan of Correction: Parents will be required to provide the allergy care plans.

Standard #: VENDSUB-000-031-B-1
Description: Based on a review of the evacuation plan, the center did not have documentation of procedures to reunite children with parents or authorized persons.

Plan of Correction: The reunification plan will be included in the evacuation procedures.

Description: Based on a review of the shelter in place procedures, the center did not have documentation of procedures to reunite children with parents or authorized persons.
