An unannounced renewal inspection was conducted today, July 17, 2017 to determine the center's compliance with licensing standards. The inspection was initiated at 2:48pm and completed at approximately 5:50pm. Upon arrival, the following ratios were observed:
Preschool room #1: 2 staff/ 7 children
Preschool room #2 : 2 staff/ 6 children
Three-Four year old room: 2 staff/ 8 children
Four-Five year old room: 1 staff/ 7 children
School age room 1: 1 staff/ 4 children
School Age room 2: 1 staff/ 5 children
Children were observed participating in arts & crafts, listening to a short story being read by staff, coloring pictures, and playing with toys. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All classrooms were inspected to include main hallways and children's restrooms. The outdoor playground was inspected with one piece of new equipment present and new resilient surfacing added.
The center had the required postings displayed near the entrance of facility and in children's classrooms. The center's first aid kit and emergency supplies were inspected with required supplies found. Eight children's and three staff records were inspected with the majority of the required information found captured, see attached notice. The following was also inspected: medication & authorizations, annual health & fire inspections, fire drill log, parent agreements, emergency preparedness plan, and other pertinent information.

Standard #: 22VAC40-185-160-C Description: Based on the inspection of staff records, the required tuberculosis (TB) test/screening update was not obtained. Evidence: During the inspection of three staff records, staff #3 had an expired TB test dated 8/27/14. This TB test expired on 8/27/16 and updated test should have been completed prior to the expiration date. Plan of Correction: Per director: The staff has received an updated TB test, however, we need to get the updated documentation.
