Comments:
An annual subsidy health and safety inspection (SHSI) was conducted from 9:00 a.m. to 12:00 p.m. with the director. Staff records were reviewed including staff training and CPR and first aid information. SHSI requirements were reviewed. Violations:
Standard #: VENDSUB-000-006-B
Description: Based on review of documentation, the center did not have documentation of a written allergy care plan for children with a diagnosed food allergy. Evidence: Child #3, has diagnosed egg, shrimp and peanut allergies and Child #4 has diagnosed peanut and egg allergies and did not have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction in the file.

Plan of Correction: The center will have a written allergy care plan which will include instructions from a physician by 7/28/17. If parents are not able to comply the children will be removed from the facility.

Standard #: VENDSUB-000-012-B
Description: Based on review of documentation, not all staff have completed Virginia Pre-service Training. Evidence: Staff #5 did not have Virginia Preservice Training.

Plan of Correction: Staff #5 will complete the Va Pre-service training by 7/28/16

Standard #: VENDSUB-000-031-B-1
Description: Based on review of documentation, the center did not have a reunification in the Emergency Plan. Evidence: The Emergency Preparedness plan did not include procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child.

Plan of Correction: Emergency Preparedness now includes all procedures for children pick up. 7/21/17
