Standard #: 22VAC40-185-270-A
Description: Based on observation, not all equipment is maintained in a safe and operable condition. Evidence: 1. Two children's rocking/chair toys were found cracked on the playground where toddlers were playing. 2. In classroom 3, 6 and 8, the child play sink is missing a part and has a hole on top of it that could cause a finger entrapment.

Plan of Correction: I removed them from the playground immediately and plan to throw away the two rocking/chair toys today. We are going to place tape or place caulking on it to plug the hole.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the physical plant, cleaning agents were found unlocked and accessible to children. Evidence: 1. In classroom 1, 2, 3, 4, 6, 8, 10 cleaning agents were found above the sink in a cabinet and were not locked. 2. In classroom 4, isopropyl alcohol and essential oils that should be out of reach of children were found in an unlocked cabinet.

Plan of Correction: We will move items to a locked cabinet immediately and we will provide a lock for any cabinets that have cleaning agents or items that need to be kept out of reach of children.

Standard #: 22VAC40-185-570-E
Description: Based on observation, the provider failed to label the infant formula with the child's name and date. Evidence: 1. One child's bottle was not labeled with the child's name on it.

Plan of Correction: They will name and date the bottle immediately.

Standard #: 22VAC40-191-60-B
Description: Based on observation, the provider failed to have an employee complete the sworn statement prior to the 1st day of employment. Evidence: 1. Three staff did not have sworn disclosures signed the first day of employment.

Plan of Correction: I had them fill the forms out immediately.
