Standard #: VENDSUB-000-031-A Description: Based on record review, staff interviews, and observation, it was determined that the facility has not completed the Emergency Preparedness plan's methods to secure the facility and designated lockdown locations. Evidence: 1. The Inspector asked a staff member where lock down procedures were kept. The staff member showed me that one room has what colors are announced for each type of emergency and a quick "what If" steps that should be taken. 2. The Emergency Preparedness plan does not include steps required to handle lock down and new staff files do not include documentation to verify Emergency Preparedness plans were discussed that addresses staff responsibility and readiness in each scenario. Plan of Correction: We have a rough draft plan but will detail plans and include in the Emergency Preparedness Plan. We will ensure documentation of this trainings recorded in the staff's files.
