Technical Assistance:
Discussed allergy action plans as it pertains to Physician's plan of action.

Comments:
An unannounced subsidy inspection was conducted in conjunction with the mandated renewal inspection. The violations were presented to the facility. Please call with questions 703-479-4679

Violations:
Standard #: VENDSUB-000-012-B
Description: Based on interviews and record review, it was determined that not all staff completed the Virginia Pre-service Training. Evidence: 1. Inspector reviewed 9 staff files and Pre-service Documentation was missing for Staff #6.

Plan of Correction: We will talk to her and will have it placed in the file.

Standard #: VENDSUB-000-031-A
Description: Based on record review, staff interviews, and observation, it was determined that the facility has not completed the Emergency Preparedness plan's methods to secure the facility and designated lockdown locations. Evidence: 1. The Inspector asked a staff member where lock down procedures were kept. The staff member showed me that one room has what colors are announced for each type of emergency and a quick "what If" steps that should be taken. 2. The Emergency Preparedness plan does not include steps required to handle lock down and new staff files do not include documentation to verify Emergency Preparedness plans were discussed that addresses staff responsibility and readiness in each scenario.

Plan of Correction: We have a rough draft plan but will detail plans and include in the Emergency Preparedness Plan. We will ensure documentation of this trainings recorded in the staff's files.
