Comments:
The licensing inspector conducted an unannounced monitoring inspection Wednesday, July 26, 2017 from approximately 10:25am to 4:00pm. The following ratios were observed with a total of 78 children present during the inspection: Infants - 9 children with 3 staff Toddlers - 6 children with 2 staff Discovery Preschool (Twos) - 16 children with 2 staff Threes- 14 children with 3 staff Pre-K - 10 children with 1 staff School Age - 23 children with 2 staff The children and staff were engaged in a variety of activities during the inspection. The Infants were observed sleeping in their cribs, playing on the floor, having their diapers changed and being fed. The Toddlers were observed having their diapers changed, looking at books, eating lunch and transitioning into naptime. The Discovery Preschool children were observed playing outside and eating lunch. The Preschool children were observed playing outside, playing in centers and eating lunch. The Pre-K children were observed playing outside. The School Age children were observed creating clay pots, playing outside and eating lunch. Lunch consisted of barbecue chicken and rolls, corn, apricots and milk. All areas of the facility including classrooms, hallways, kitchen, playgrounds, and bathrooms were inspected. The facility was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports and injury prevention plan, daily sheets, daily attendance, infant feeding plans, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The center's first aid kit and emergency supplies were inspected. Medication is administered and medication and medication authorization forms were reviewed. Transportation is provided and vehicles and transportation documents were reviewed. During the inspection, seven children's records and seven staff records were reviewed. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of seven children's records, one record did not contain the required documentation of immunizations. Evidence: During the inspection, the licensing inspector observed the record for Child #6 (start date 07/24/17) did not contain documentation of immunizations. Immunization records are required to be obtained prior to the child attending the center.

Plan of Correction: The parent has already been notified that the immunization record is required. We will add the information to the child's record once it is received.

Standard #: 22VAC40-185-60-A
Description: Based on review of seven children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #2 (start date 3/29/17) did not contain the address for one of the two emergency contacts listed.

Plan of Correction: The parent was notified during the inspection and the address will be added to the record.

Standard #: 22VAC40-185-260-A
Description: Based on documentation review, the facility did not ensure an annual fire inspection was completed by the fire official. Evidence: During the inspection, the licensing inspector observed the most recent fire inspection was conducted on 02/26/16.

Plan of Correction: The local fire department was contacted during the inspection. A fire inspection is scheduled for tomorrow.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not ensure all areas were maintained in a safe and operable condition.

Evidence:
During the inspection, the licensing inspector observed an approximate one inch rusty screw protruding from the wall behind the toilet in the Twos classroom.

Plan of Correction: During the inspection tape was placed over the screw. A work order will be submitted for a permanent solution.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed a can of air freshener in an unlocked cabinet over the diaper changing table in the Twos classroom. The can was labeled "Caution: Keep out of the reach of children".

Plan of Correction: The air freshener was moved to a locked location while the licensing inspector was on site.
