An unannounced monitoring inspection was conducted in the Center on July 26, 2017 beginning at 2:00 pm. There were 28 children present with 4 caregivers. The children were observed in two separate buildings. Reportedly, 36 children are currently enrolled. A variety of activities were observed which included nap/rest time, hand-washing procedures, snack, outdoor play and a teacher-directed circle activity. Dismissal procedures were observed at the end of the program day. A sample of 5 children's records were reviewed. Staff records are maintained at the central office, 7 staff records to include support staff were reviewed. Prescribed medications were observed for 4 children. Five staff have documentation of current medication administration training. Documentation of the practiced emergency drills was reviewed. Reminder: an evacuation drill is due for the month of July. Standard #: 22VAC40-185-150-B Description: Based on documentation reviewed, the Center failed to ensure a child's immunization record was signed by a physician, his designee, or an official of a local health department. Evidence: One of five reviewed children's records, child #3, did not contain a signed immunization record. The reviewed document was dated 10/14/15 but was not signed by a physician or his designee. Plan of Correction: The Family Development Specialist will follow-up with the child's parent and will request a signed immunization record for record retention.
Standard #: 22VAC40-185-160-C Description: Based on a review of staff records, the Center failed to obtain documentation of a tuberculosis screening at least every two years from the date of the first initial screening. Evidence: Documentation of the TB screening for staff member #7 was not current within two years, the screening was dated 6/2/15. Plan of Correction: Staff will be notified to schedule an appointment for the TB screening, Current documentation will be on file.
