Standard #: 22VAC40-185-150-B
Description: Based on documentation reviewed, the Center failed to ensure a child's immunization record was signed by a physician, his designee, or an official of a local health department. Evidence: One of five reviewed children's records, child #3, did not contain a signed immunization record. The reviewed document was dated 10/14/15 but was not signed by a physician or his designee.

Plan of Correction: The Family Development Specialist will follow-up with the child's parent and will request a signed immunization record for record retention.

Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, the Center failed to obtain documentation of a tuberculosis screening at least every two years from the date of the first initial screening. Evidence: Documentation of the TB screening for staff member #7 was not current within two years, the screening was dated 6/2/15.

Plan of Correction: Staff will be notified to schedule an appointment for the TB screening, Current documentation will be on file.
