An unannounced monitoring inspection was conducted on July 27, 2017 from approximately 10:10 a.m. to 2:35 p.m.. There were 6 staff were present with 34 children. The sample size of records reviewed were 5 children's and 5 staff records on this date. The center is not using the outdoor playground. Children were observed working on counting projects and bible stories. Areas of noncompliance are identified on the violation notice. Standard #: 22VAC40-185-240-D-4 Description: Based on observation and staff interviews, the licensee did not ensure that any child for whom emergency medications (such as but not limited to albuterol, glucagon, and epipen) have been prescribed there shall always be in the care of a staff member or independent contractor who has satisfactorily completed a training program for this purpose approved by the Board of Nursing and taught by a registered nurse, licensed practical nurse, doctor of medicine or osteopathic medicine, or pharmacist; or independent contractor who is licensed by the Commonwealth of Virginia to administer medications. Evidence: During the inspection on July 27, 2017, there were two children in care who require emergency medications and there was not a person on site who has successfully completed the training to administer medication as required by the licensed child day center standards. Child #5 has an Epipen on site and child #6 has an Epipen and Albuterol on site. The center director confirmed child #5 and child #6 were on site and the medications would be used in in an emergency situation. Plan of Correction: The provider will send home all medication and will not keep any on site.
Standard #: 22VAC40-185-510-J Description: Based on observation and staff interviews, the licensee did not ensure medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection on July 27, 2017, there were 8 medications observed in an unlocked filing draw in the office. The medications included 4 Epipens,1 Albuterol container, 1 container of Diphehydramine and1 container of Children's Benedryl. Plan of Correction: The center director will send medication home of dispose of it.
Standard #: 22VAC40-185-510-N Description: Based on observation and staff interviews, the licensee did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. Evidence: During the inspection on July 27, 2017, there were 2 medications for child #6 has an Epipen and Albuterol and the physicians authorizations and parent authorizations had expired on 6-30-2016 and the medications had not been sent home to the parent within 14 days of expirations. Plan of Correction: The center director will send medication home of dispose of it.
Standard #: 22VAC40-185-560-F Description: Based on observation and staff interviews, the licensee did not ensure that when centers choose to provide meals or snacks, the center must follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: During the inspection on July 27, 2017, the menu for the afternoon snack was reviewed and it listed that yesterday, (July 26, 2017) the children were served a poptart and water. This was insufficient as USDA guidelines were not met because only two of the four food groups were served. The menu for today's afternoon snack list rice kripsy treats and water and tomorrow's afternoon snack is listed as cookies and water. The center director confirmed the menu and that children were served only poptarts and water for afternoon snack on Wednesday July 26, 2017. Plan of Correction: The center director will add fruit to the afternoon snacks and will meet the USDA guidelines in the future.
