Comments:
An unannounced Health, Safety and Subsidy inspection was conducted on August 1, 2017 at 9:15 a. m. and concluded at 12:15 p. m. The inspector met with the staff and Child Day Center subsidy requirements were reviewed. The following staff-to-child ratios and activities were observed. Infants: There were three infants with one staff. Older Infant/Toddlers: There were four toddlers with one staff. Two Years: There were eight children with one staff. Preschool (Three Years) There were nine children with one staff. The children were observed playing outside on bikes and having free play. Pre-K (Four-Five Years): There were seven children with one staff. The children were observed having free play outside on the playground. School Age: There were 15 children with two staff. The children were sitting at tables playing games. Four children's records were reviewed. Four staff records were reviewed. Two classroom observations were conducted. Conversations were held with several staff and children. There were no medications on site today. The last monthly evacuation drill was conducted on 7/5/17.

Standard #: VENDSUB-000-012-F
Description: Based on review of staff files the center did not have all staff with required training. Evidence: On 8/1/17, staff #1(hire date 8/17/15) did not have current First Aid and CPR.

Plan of Correction: A First Aid and CPR class is being offered to all staff on 8/8/2017.
