Standard #: 22VAC40-185-40-J Description: Based on a review of administrative documents, it was determined that the injury prevention plan has not been reviewed as required. Evidence: There is no documentation showing that the injury prevention plan has been reviewed annually. Plan of Correction: I will remember to document each annual review.
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #6 does not include: authorized pick ups information; a shot record; a physical exam report. Plan of Correction: Camp ends next week. The parent will be asked for the missing information.
Standard #: 22VAC40-185-280-B Description: Based on observations, it was determined that two cleaning products were not locked. Evidence: Cleaning products in the Drama classroom and in the Art classroom were not stored in locked cabinets. Plan of Correction: I will ask the janitor to move the teachers items to the locking cabinets.
Standard #: 22VAC40-185-510-A Description: Based on a review of children's medications, it was determined that one child's medication does not match the instructions on the authorization form. Evidence: One child's medication authorization form indicates that up to two injections of the medication must be given in an emergency. However, only one injection of the medication is available on site. Plan of Correction: Camp ends next week. The parent will be asked to bring a second medication.
