Discussion was held on the topics of current enrollment, current staffing, subsidy, finger print requirements, cps forms and sds forms.

Comments:
The following inspection took place at 8:30 am on on the morning of 08/02/2017, and on 08/08/2017 between the hours of 12:30 pm through 3:30 pm. Standard #: 22VAC40-185-350-C
Complaint related: No
Description: Based on review and observation, a regularly ongoing mixed age group, the staff-to-children ratio applicable to the youngest child in a group was not met for the entire group. Evidence - 1. On the first date of inspection (08/02/2017), at approximately 8:30 am. there were 22 children with 2 staff in the kindergarten room. The age range of the children in care ranged from 4 years up to 11. 2. Based on the age of Child #1 (4 years of age), the correct ratio should have been 1 staff for every 10 children. 3. On the second date of inspection (08/08/2017), the Center Director confirmed that the youngest child in the room on that date and at that time would have been Child #1. 4. The Center Director stated (during an interview conducted on the second date of inspection) that it was her understanding that since this child was going to attend the center's kindergarten program in the fall that it would be permissible.

Plan of Correction: An additional staff person shall be placed in the room, or a parent permission and written developmental assessment form may also be completed.

Description: Based on observation and interview, food stored in insulated containers did not include ice packs to keep the food cold. Evidence - 1. On the first date of inspection (08/02/2017) at approximately 8:30 am., food (student lunches) were observed in an open cooler that was sitting on a floor in the school-age room. 2. Staff #1 (stated in an interview conducted on the first date of inspection) that the lunches go into the cooler once the classroom refrigerator is full. This same cooler was to be used for these same lunches at the start of the scheduled field trip at 10 am. However, no ice packs were included. When asked why couldn't these lunches be placed in the center refrigerator located in the kitchen, Staff #1 stated that she too asked the same question to the administration.

Plan of Correction: Ice packs shall be used and a later time shall be administer for the start of packing lunches for the field trip. If needed the kitchen refrigerator shall also be used.
