Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on August 8, 2017 from approximately 10:30 a.m. until 3:45 p.m Upon entrance there were 79 children and 11 staff present with children. There were additional staff on site for administrative and program purposes. The record review sample consisted of 10 children's records and 30 staff records. Children were observed freely playing in the centers to include homeliving, cars, trucks and books. The schooalge children were observed during a gymnastics program.

Violations:
Standard #: VENDSUB-000-004-F
Description: Based on observation and vendor interviews, the vendor did not ensure that with parental approval, or kept in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions was posted. Evidence: During the inspection August 8, 2017, in classroom #104 there was list of children's food sensitivities posted on the classroom bulletin board and vendor confirmed parent permission had not been obtained to post this information. The names of child #5 and child #6 were posted on the classroom bulletin and it said "no milk, pineapple, orange".

Plan of Correction: The vendor will remove the list.

Standard #: VENDSUB-000-012-E
Description: Based on observation and interviews, the vendor did not ensure all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval CPR and first aid training. Evidence: During the inspection on August 8, 2017, the following staff did not have documentation of obtaining CPR and first aid trianing. a) staff #1 DHO (date of hire) 6-25-17 b) staff #2 DHO 7-7-17 c) staff #5 DHO 4-3-17 d) staff #6 DHO 7-7-17 e) staff #10 DHO 1-23-17 f) staff #11 DHO 5-25-17 g) staff #14 DHO 6-7-17 h) staff #17 DHO 11-17-15 has documentation of first aid but not CPR i) staff #24 DHO 5-19-17 j) staff #25 DHO 5-19-17 k) staff #26 DHO 5-19-17

Plan of Correction: The vendor will have schedule the training and ensure that the staff have it.

Standard #: VENDSUB-000-031-B-1
Description: Based on observation and interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an emergency to include evacuation, shelter-in-place and lockdown emergencies. Evidence: During the inspection on August 8, 2017, there were not any reunification plans documented in the emergency preparedness plan as confirmed by the vendor.

Plan of Correction: The vendor will correct it.
