COMPLAINT # 47818 An unannounced complaint investigation took place at the center today. Staff were interviewed and records were reviewed. The following standards were examined as part of the complaint: 270 A: Physical environment; 270 5 E: Supervision; 350 F: Staff - ratio requirements. After careful review, it was determined that the following was not in compliance: 350 F: Reassignment of children Though not part of the complaint, a standard was found to be in violation during the course of the investigation. 500 B 6: Diapering procedures. Standard #: 22VAC40-185-350-F Complaint related: Yes Description: Based on a review of records and staff confirmation, children have been reassigned from their regular groups and staff members on a repeated basis this summer in a manner that that did not follow the standards for reassignment. Evidence: While a center may temporarily reassign a child from his regular group and staff for reasons of administrative necessity, it cannot be done casually or on a repeated basis that disrupts a child's schedule/ attachment to staff/classmates. According to attendance lists and staff, infants and toddlers were regularly changed from Infant B group to Infant A group in the early morning hours 7- 8:00 and late afternoon hours on dates in June and July. On June 23, the attendance form indicates that Infant # 1 went into Infant B, then Infant A, and later Infant B. Infant #2 went from Infant A - B; and Infant #3 went from Infant B to Toddler A. From 10 - 1, Infant # 4 went to Infant A. At 4 pm, the same was indicated for Infant # 4 and 5. Additionally, five younger school-agers were placed with the pre K group from 11:44 am - 3:14 pm. Staff state that this was at the request of parents, but there was no written permission and written assessment available as required. Plan of Correction: We will get written permission from parents and staff for younger school-age children to rest in the pre K class after lunch if that is was parents would like. We will rearrange the morning and late day schedules to avoid repeated reassignment of children.
Standard #: 22VAC40-185-500-A Complaint related: No Description: Not part of the complaint but observed during the inspection: Two diapers were observed to be disposed of in a manner that did not prevent the spread of germs. Evidence: A staff member was observed to be using hands to dispose of soiled diapers in the foot-operated diaper trash disposal . Plan of Correction: We will retrain as necessary.
