Comments:
An unannounced monitoring inspection was conducted from 10:00 a.m.-11:20 a.m. with the director. 23 children, ranging in age from 22 months through 5 years old, were under the care and supervision of three staff. Children were observed building with Legos, reading, drawing, and eating morning snack. Morning snack served today was mixed cereal and fruit juice. Required information was posted. Both the fire and health inspections are current. There were no new medications to review. Five children's records and one new staff record were all reviewed. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance. Evidence: Child A's record did not have documentation of a physical.

Plan of Correction: Child A's parents had brought in another copy of the immunization record rather than the physical. A copy of the physical will be requested again at pick-up today. The director will give the parents a copy of the physical examination form.
