Comments:
Today an unannounced self-reported visit was conducted with School Principal regarding medication procedures and a child given the wrong medication dosage. This visit took place in the morning between 11:30-2:15pm there were 90 children with 18 staff within the supervision guidelines. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interview with Staff and review of medication forms, center failed to provide care, protection and guidance when a child was giving the wrong dose of medication. Evidence: On July 24th Child #1,(2 years old) was given the wrong dosage of medication based on the prescription label and the doctors note. The prescription label and the medication consent form did not reflect the correct dosage amount and therefore Staff failed to ensure care, protection and guidance when they failed to review the medication consent form and prescription label to ensure that the information matched and reflected the correct information.

Plan of Correction: Principal stated that a review of the MAT 5 rights will be reviewed by all MAT trained staff. Principal stated all staff will be required to signoff on the new medication policy for the school. Principal also stated that a verifier was added to the MAT form and must be signed off by another MAT staff.

Standard #: 22VAC40-185-510-E
Description: Based on review of medication labels and interview with staff, a medication label was found that did not reflect the correct dosage amount to be given. Evidence: On July 24th Child #1,(2 years old) was given the wrong dosage of medication based on the prescription label and the doctors note. The prescription label and the medication consent form did not reflect the correct dosage amount and therefore Child #1 was given the incorrect dosage. Staff failed to give the correct dosage amount and to follow the correct time to be given.
