Comments:
An unannounced monitoring inspection was conducted from 10:45 am - 1:45 pm. There were 73 children in care being supervised by fourteen staff members. The children were observed participating in cooperative games, table top activities, outdoor play, and eating snack. All meals are provided from home. Eight staff records were reviewed. Ten children's records were reviewed. Four medications were reviewed and in compliance. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on observation of ten children's records it was determined that not all records contained a physical within the first 30 days of enrollment. Evidence: Child A with a start date of 2/22/16 did not have record of a physical on file.

Plan of Correction: Administration will request a copy of the most recent physical from the parents of Child A.

Standard #: 22VAC40-185-240-D-1
Description: Based on interview with administration it was determined that not all staff obtained the required Medication Administration Training for specific medications on site. Evidence: An emergency medication is on site which would require staff to be certified in Rectal Medication Administration Training. There are no staff on site during the inspection that would be certified to administer the medication if necessary.

Plan of Correction: Administration will require staff to obtain the required Medication Training.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the specialty room it was determined that not all areas of the center were maintained in safe and operable condition. Evidence: A plastic container holding miniature cars and trucks located in the specialty room was broken with sharp edges exposed and accessible to children.

Plan of Correction: Administration removed the container from the classroom during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation of the playground it was determined that the appropriate amount of resilient surfacing was maintained around playground equipment. Evidence: The areas under the slide equipment were bare down to the dirt and did not have 6 inches of resilient surfacing.

Plan of Correction: New mulch is on site and will be added to the playground equipment areas fall zones.

Standard #: 22VAC40-185-500-B
Description: Based on observation of the two year old classroom it was determined that not all procedures for diapering were followed. Evidence: A separate lined, foot operated trash system was not used for children using cloth diapers.

Plan of Correction: Administration will provide a separate foot operated trash can with a liner for each child using cloth diapers.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on observation of eight staff records it was determined that not all staff repeated background checks every three years. Evidence: The most recent Central Registry Check for Staff A was dated 02/24/2003.

Plan of Correction: Administration will submit a central registry check for completion for Staff A.
