Comments:
An unannounced monitoring inspection was conducted today from 10:15am-11:45am. There were 18 children directly supervised by 3 staff. The physical plant, 3 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating baking pumpkin muffins, playing outdoors and with various Montessori manipulatives. Teachers were observed encouraging children to wash hands. Positive interactions were observed with teachers using calm and respectful words and tone of voice. There was an abundant supply of materials for the children. The center was clean and organized. No medications were present during today's inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Based on record review, one staff person did not submit updated TB test within two years from the last test. Evidence: Staff 3 last TB screening was completed 12/2014.

Plan of Correction: Requested an updated TB test.

Standard #: 22VAC40-185-280-H
Description: Based on observation, purses with cosmetics were accessible to children. Evidence: 1. Two staff purses were on a hook in the hallway, accessible to children. 2. Owner of purse demonstrated that there were cosmetics in the purse.

Plan of Correction: Staff will ensure that cosmetics are removed from bag each day and kept in locked place.

Standard #: 22VAC40-185-340-F
Description: Based on observation, children under ten years of age were not always within actual sight and sound of staff. Evidence: 1. Children were observed transitioning from classroom to outdoors via a hallway that was not within sight and sound of any staff member. 2. Director demonstrated that children use a system to check out of the classroom before transitioning outside. 3. The unsupervised hallway has access points to outside into parts of the building not operated by the child day center.

Plan of Correction: Staff will move transition area within sight and sound of staff positioned in the classroom. When walking down the hall a staff person will be positioned at doorways to keep children within sight and sound.

Standard #: 22VAC40-185-510-J
Description: Based on observation, not all medications were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. A wall mounted first aid kit contained Ibuprofen and Antacids. 2. The first aid kit was latched but not locked. 3. The first aid kit was mounted five and half feet off the floor but still accessible to the children.

Plan of Correction: Medications were removed during inspection.

Standard #: 22VAC40-185-560-G
Description: Based on observation, lunches brought from home were not clearly dated and labeled in a way that identifies the owner. Evidence: All lunch bags were observed without names or dates clearly labeled.

Plan of Correction: We will resume labeling the lunches from home.
