Comments:
The licensing inspector conducted an unannounced renewal inspection Wednesday, July 26, 2017 from 1:58pm to 6:00pm. The children were observed resting on mats. They were later observed getting snack and being picked up by their families. The infants were observed sleeping in cribs, being held and being fed. The school age class was on a field trip, and later were observed returning to the center. Snack consisted of lemon and blueberry cookies and milk. The menu was posted. All classrooms and playgrounds were inspected today. Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 07/19/2017 Shelter-in-place drills: 04/06/2017 Last fire inspection: 04/11/2017 Last health inspection: 05/30/2017 Total enrollment today: 51 Today, the following child to staff ratios were observed: Infant I ?4:1 Infant II (young toddlers)-5:1 (restime ratio) Toddlers ?6:1(rest time ratio) Twos ? 14:2 (rest time ratio) Threes ?16:1 (rest time ratio) Pre-K ?18:1 (rest time ratio) School Age ?16:1 The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. The Department has not yet received a complete renewal application and annual fee from the center. The Department must receive a complete renewal application and fee prior to expiration of the license on September 4, 2017 or the Department cannot renew the license. If you have any questions about this inspection, please contact your licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today (August 2, 2017). You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on a review of staff records on 07/26/2017, the center did not ensure that one of five records reviewed contained documentation of a negative tuberculosis(tb) screening within 12 months prior to employment. EVIDENCE: The tb screening in the record of Child #1, whose date of hire was 05/15/2017, is dated 11/13/2015.

Description: Based on a review of children's records on 07/26/2017, the center did not ensure that five three of five records reviewed contained the required information. EVIDENCE: 1. The most recent update of Child #1's record is dated 5/18/2015. 2. The most recent update of Child #2's record is dated 01/27/2016. 3. The record of Child #3 did not contain the telephone number for the place of employment listed for Child #3's mother.

Description: Based on observation on 07/26/2017, the center did not ensure that hazardous substances such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. EVIDENCE: 1. A spray bottle labeled "bleach water" was observed in an unlocked closet in the preschool classroom 2. A spray bottle labeled "bleach water" and other cleaning products were observed in an unlocked cabinet in the lower school age classroom.

Description: Based on observation on 07/26/2017, the center did not ensure that on the school age playground the resilient surfacing shall comply with the minimum safety standards. EVIDENCE: There was less than 6 inches of mulch under the climbing and exercise equipment on the trail area of the school age playground.
