- Technical Assistance:
The playground was discussed with the program director. One corner of the playground seems to accumulate litter, blown into the playground from the adjoining alleyway. The playground must be checked by staff prior to every use for removal of any trash/littler. The program director was reminded that the declarations page from the current liability insurance coverage must be available for review.
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 2:30 PM and departed at 4:35 PM. At the start of the inspection, eighteen children were in care with three staff and the program director. A sample size of six staff records and five children's records were reviewed. Injury records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.
Standard #: 22VAC40-185-40-J Description: Based upon review of the injury prevention plan, the facility has not ensured that the injury prevention procedures are updated at least annually based upon the documentation of injuries and a review of the activities and services. Evidence: The facility's injury prevention plan indicates a review date of 6/1/2016. There is no indication of an update since 6/1/2016. Plan of Correction: The facility responded with the following: The injury prevention will be reviewed and updated at least annually. The review will be dated/documented.
Standard #: 22VAC40-185-280-B Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children. Evidence: The child proof latch on the high cabinet on the wall across from the bathroom is broken. There was a container of bleach and a container of pine cleaner (and other cleaning products), labeled "warning, keep out of reach of children" in the cabinet. Plan of Correction: The facility responded with the following: A new child proof latch will be placed on the cabinet. In the interim, the cleaning and other hazardous materials will be moved to locked locations.
Standard #: 22VAC40-185-500-B Description: Based upon observation, the facility has not ensured that the diaper changing station includes a non-absorbent surface. Evidence: The diaper pad on the changing table in the first bathroom is torn and thus is no longer non-absorbent and cannot be a sanitized as required.. Plan of Correction: The facility responded with the following: The changing pad will be replaced. Staff will be reminded to notify management when the pads begin to tear.
