Comments:
An unannounced monitoring inspection was initiated today at 12:45pm. The physical plant, 5 staff files, 5 children's files, annual fire inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 20 children under the supervision of 3 staff. During the inspection, children were observed reading books, listening to music, and resting during naptime. The children bring in their lunches from home daily. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 2 staff certified in MAT (medication administration training). Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Standard #: 22VAC40-185-60-A-7
Description: Based on file review, 2 out of 5 children's records has a food allergy listed without what actions to take in an emergency situation. Evidence 1. Children #2 and #5 have food allergies listed on their paperwork. There is written information that medications are not needed, but there is no plan of action if the children were to consume these foods.

Plan of Correction: The assistant director will communicate with the parent about getting a written plan of action.

Standard #: 22VAC40-185-330-B
Description: Based on observations, there was an insufficient amount of mulch within the fall zones of playground equipment. Evidence 1. The tire swing and climbing structure with slide had less than 6 inches of mulch within the fall zones.

Plan of Correction: We will redistribute the existing mulch in the playground to these areas.

Standard #: 22VAC40-185-560-G
Description: Based on observations, the food containers brought from home did not meet all requirements. Evidence 1. There were food containers children brought their lunches in from home, which were not clearly labeled with the date.

Plan of Correction: The assistant director will discuss this with the Director to see if the parents or staff will be asked to complete this daily.
