Licensing inspectors arrived at 9:37am and departed at 11:45am to conduct an unannounced monitoring inspection. During the inspection there were 30 children ages ten months to six years old in care with eight staff. Children were observed eating lunch, participating in outdoor activities, and various classroom activities. The sample size was five children's records and five staff records. Areas of non-compliance are identified on the violation notice, and were in the area of emergencies procedures, nutrition and food services, transportation. The violations were discussed during the exit interview with the Program Director.
Standard #: 22VAC40-185-550-H Description: Based upon observation, it was determined that the center did not prepare a document containing a local emergency plan of frequently visited routes driven by center staff for pick up/drop off of children to or from school. The document was not kept in the vehicle that the center uses for transportation of children. Evidence: 1. The van used to transport children to and from the center on a daily basis did not have a complete emergency plan on the vehicle that is used for transportation of children. 2. Staff #5 (Program Director) reviewed the transportation book for the van, and confirmed that plan present did not contain all of the required items. Plan of Correction: The facility responded: An emergency form will be made and placed in the van for transportation to field trips, home pick up/drop off and school drop off / pick up.
Standard #: 22VAC40-185-560-F Description: Based upon observation, it was determined that the center did not ensure that a menu listing for foods to be served for meals and snacks was not dated correctly for the current week. Evidence: 1. The menu posted in the lobby of the center that listed the meals and snacks to be served was dated 8/14/17 - 8/18/17. 2. Staff #5 (Program Director)confirmed that posted menu for the center was not dated for the current week of 8/21/2017-08/25/2017. Plan of Correction: The facility responded: The menu will be posted every Monday for the current week.
Standard #: 22VAC40-185-560-G Description: Based upon observation, it was determined that the center did not ensure that when food is brought from home it was properly labeled with the a date. Evidence: 1. The bottle of milk used for Child #4 located in the refrigerator in the infant room was not properly labeled with the date. 2. Staff #4 confirmed that the bottle for child #4 was labeled with the date. Plan of Correction: The facility responded: Staff will be reminded that when food, bottles or cups are brought from home they shall be labeled with the child?s name and dated.
Standard #: 22VAC40-185-560-J Description: Based upon observation, it was determined that the center did not ensure that expired or out dated food was not used for nutrition and food services for children. Evidence: 1. In the Infant room, there was a can of powered formula that was labeled with the date of the can being opened on April 30 2017. The label on the can read once the can has been opened, use the content (formula) within one month. The date of the inspection was 8/23/2017. 2. Staff #5 (Program director) confirmed that the can was expired and should not be used according the the label on the can. Plan of Correction: The facility responded: All extra formula and food for infants will be labeled with a throw away date and will be discarded as required. Extra formula was thrown out during inspection.
Standard #: 22VAC40-185-580-B Description: Based upon observation, it was determined that the center did not ensure that the vehicle meet the safety standards set by the Department of Motor Vehicles. Evidence: 1. Two booster seats located on the van used of transporting children to and from the center on a daily basis were labeled with the expiration dates to no longer use after 2016 and 2013. The date of the inspection was 8/23/2017. 2. Staff #5 (Program Director) confirmed that two of the booster seats used on the van were expired and should not be used. Plan of Correction: The facility responded: The two expired booster seats were thrown away and replacement booster seats will be purchased.
