A Subsidy Health and Safety inspection was conducted on Wednesday, August 30, 2017, from approximately 11:00am until 4:30pm. to review supplemental health and safety requirements.
The following classroom ratios were observed:
Infants: Ten children with three staff
Room 1 - Toddlers - Six children with two staff
Room 2 - Two's - Eight children with two staff
Room 3 - Two's - Eight children with one staff
Room 4 - Two's - Eight children with one staff
Room 6 - Three's- Nine children with one staff
Room 7 - Three's- Eight children with one staff
Room 8 - Pre-K - Four children with one staff
Kindergarten - 2nd- Seventeen children with one staff
2nd- 5th - Eighteen children with one staff
Observations were made by the licensing inspector during outdoor play, during classroom activities, lunch time and nap time.
Children in the school age building have their summer time art posted which includes "Sparkle Name Art", Canvas Paintings, and a mural of James River. They were preparing to play Family Feud, and GaGa Drill.
Infants were being held, having bottles and floor play. Preschool children were playing dress-up, building with blocks and legos, and signing along to "ABC Mouse".
The last practice evacuation drill was conducted 8/23/17.
Lunch: Beef tacos with cheese, corn, peaches and milk
Children's medications and food allergies were checked for compliance.
Ten children's records and ten staff records were inspected.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.
Standard #: VENDSUB-000-012-B Description: Based on review of staff records, the center did not ensure staff completed the Virginia Preservice Training for Child Care Staff within the first 90 days of employment or subsidy vendor approval which was required to be completed by March 31, 2017. Evidence: Out of ten staff records inspected: Staff #4, started 8/5/95 and did not complete the required training until 4/18/17. Staff #5, started 6/1/10 and did not complete the required training until 8/19/17. Staff #6, started 10/13/15 and has not completed the required training. Staff #8, started 10/23/14 and did not complete the required training until 6/9/17. Staff #9, started 8/25/14 and did not complete the required training until 6/8/17. Staff #10, started 4/12/10 and did not complete the required training until 8/24/17. Plan of Correction: Per Center: " During routine audit it was discovered the training was late. It has been or will be corrected."
Standard #: VENDSUB-000-012-E Description: Based on review of staff records, the center did not ensure all staff received certification in first aid and CPR within 30 days of employment. Evidence: Staff #1, started 7/12/17, and does not have certification in first aid or CPR. Staff #3, started 3/2/17, and does not have certification in first aid or CPR. Plan of Correction: Per Center: " During a routine audit it was discovered the training was late. It will be corrected."
