Discussed the Director uncovered outlets in the assembly area as well as Comet cleaner on the counter in the kitchen. It was also suggested to have the mulch on the playground raked and more evenly dispersed.

An unannounced renewal inspection was conducted on 09/01/2017. The inspections began at 9:30am and ended at 1:00pm. There were three staff with 15 children in one classroom. The children were observed playing quiet games on the floor and going outside to play. The Kindergartners were observed coming in from the school bus. Positive interactions were observed. All of the children provided their own lunches and the scheduled afternoon snack was Oreo cookies. A complete inspection of the physical plant and playground was conducted. Children's files, injury reports, emergency preparedness, and playground safety were reviewed today. Thank you for your cooperation. Please email with questions:

Description: Based on observation, areas of the outside playground was not maintained in a clean, safe and operable condition. Evidence: There are two climbing structures on the 5-12 year old play structure that has chipped paint and rust.

Plan of Correction: I will ask to have it painted. I am currently in negotiation to get a new playground.

Description: Based on record review, children's accident and injury reports did not contain all of the required information. Evidence: 8 out of 15 accident/injury reports did not contain the time that the parents were notified on the accident/injury.

Plan of Correction: Staff members will be reminded to fill this portion of the form before handing it in.
