Description: Based on observations, the facility did not ensure all areas were maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed the following on the outside grassy play area: 1. A basketball goal contained torn netting that hung down, creating a head entrapment hazard. 2. A section (two feet by one foot) of potentially hazardous wild mushrooms were observed in the play area to be used by the children.

Plan of Correction: Both the netting and the wild mushrooms were removed from the play area.

Description: Based on observation and interview, the facility did not ensure all electrical outlets contained protective covers.

During the inspection, the licensing inspector observed two electrical outlets located in both the male and female bathrooms that did not contain electrical covers. A staff member stated additional electrical outlets needed to be purchased.

Plan of Correction: We will buy the covers and place them in the electrical outlets.
