An unannounced monitoring inspection was conducted on September 11, 2017 between the hours of 10:18 a.m. and 2:30 p.m. Forty-two (42) children were in the care of six staff members during the on site inspection. One additional staff member was present and able to assist, as needed. Five children's records were reviewed and five staff records were reviewed and/or updated. Three children's medications and three children's topical ointments, as well as authorization forms were reviewed. Program activities, daily routines, interactions with children, staffing, supervision, equipment, posted information, one agency inspection report, injury records, emergency supplies and drill records, all classrooms and the outdoor playground areas were observed/reviewed. Lunches and snacks were provided by the children's parents. The facility was observed to be clean and supplied with a variety of age/stage program specific learning and play materials. Program observation was completed. Activities observed on this date included: clean-up and group time in the 2 and 3-year-old classroom (morning greeting, songs and story); outdoor play; handwashing and restroom procedures; lunch; naptime and departure/pick-up of children. Staff/child interactions, behavior guidance and daily routines/transitions were observed. Staff members were observed providing attentive supervision during activities and transitions. Previously cited violations were reviewed for compliance; no repeat violations were cited. Refer to the violation notice for the exception to compliance. Standard #: 22VAC40-185-160-C Description: Based on review and/or update of five staff records and discussion with staff on September 11, 2017, the program failed to ensure one staff member submitted the results of a follow-up tuberculosis screening at least two years from the date of the first initial screening or testing. Evidence: 1. Update of staff #1's record, on this date, determined that the most recent TB screening was completed on 8/31/2015. 2. Staff #1 verified the date of the most recent TB screening report. Plan of Correction: An updated TB screening report will be obtained.
