Technical Assistance:
Discussed placing names on infant bottles instead of the tops of the bottles. The fire Marshall is scheduled for 10/19/17 to update permit, the fire inspection is current.

Comments:
An unannounced monitoring inspection was conducted from 9:15 am-12:00. At the time of entrance, 51 children were in care with 12 staff members present. Children were observed during snack time, engaged in group activities; puzzles, matching games, math, writing words, hand washing and diapering. The playground and surfacing was inspected. A selection of staff and children records, medications and other documents were reviewed. The Director contacted DSS during this inspection to cancel the subsidy contract. The contract was cancelled effective 8/31/2017. A subsidy visit was not conducted this date. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-160-C
Description: Based on record review, the provider has not obtained the results of a follow-up TB screening every 2 years. Evidence: Staff #1 TB screening was dated 5/11/15, not within the last 2 years.

Plan of Correction: I will have the staff update her TB screening.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the provider has not maintained the required resilient surfacing under outside play equipment and fall zones. Evidence: The ground was found bare around and at the bottom of the slide on the small playground. The ground was found bare around and at the bottom of the slides and red ladder on the large playground.

Plan of Correction: I will move pea gravel and add additional pea gravel to those areas.
