An unannounced monitoring inspection was completed on 9/8/17. There were 25 children present with two staff supervising. Five children's records and two staff records were reviewed. Three emergency medications were observed for one child. The inspector arrived at the school at 4:00 pm and departed at 6:00 pm. The children were observed during snack and movie time, during table toy/free time, and during departures. There was discussion with the site director about the following: location/storage of cleaning supplies, food sensitivity postings (subsidy requirement), upcoming fire inspection due (October).
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that two of five children's records contained documentation of all required information. Evidence: 1. The father of child #1 was listed as not authorized to pick up the child. There was no legal paperwork to support this. 2. The record of child #5 was missing a complete address for two of two required emergency contacts for the child. Plan of Correction: The site director will get the information documented in the children's files and get custody orders for any legal issues of authorized pickups.
Standard #: 22VAC40-185-550-C Description: Based on observation of the posted emergency evacuation and shelter-in-place map, the center failed to ensure that the shelter-in-place location was noted on the map. Evidence: 1. The posted emergency map was observed. There was no shelter-in-place (tornado) location noted on the map. Plan of Correction: This was corrected during the inspection. The staff noted the shelter-in-place location on the map.
Standard #: 22VAC40-185-550-D Description: Based on observation of the posted emergency evacuation log and interview with staff, the center failed to ensure that there was a monthly emergency evacuation drill (fire drill) practiced. Evidence: 1. There was no documentation of a fire drill that was practiced in August 2017. The program started operation on 14, 2017. 2. According to staff, they did not practice a drill for the month of August. Plan of Correction: A drill will be practiced monthly from this month forward.
