Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on September 7, 2017 from approximately 8:30 a.m. until 1:05 p.m. Upon entrance there were 8 children and 4 staff present with children. The record review sample consisted of 5 children's records and 5 staff records. Children were observed eating snack, jumping singing and dancing. Please contact the licensing inspector with any questions (757) 404-0678. The results of the inspection were reviewed and verified with the center director/owner.

Violations:
Standard #: VENDSUB-000-004-F
Description: Based on observation and interviews, the vendor did not ensure that parental approval had been obtained when posting a current list of all children?s allergies, sensitivities, and dietary restrictions or the list was kept in a way that is immediately accessible to staff. Evidence: On September 7, 2017, there was a list posted in the cafeteria with two children's names (child #6 and #7) with their food allergies/sensitivities visibly listed and in the open kitchen area there was a list posted with five children's names (child #1, #2 ,#3, #6 and # 7) with their food allergies/sensitivities visibly listed and parental permission had not been obtained.

Plan of Correction: The vendor covered list during the inspection.

Standard #: VENDSUB-000-012-D
Description: Based on interviews, the vendor did not ensure orientation training for staff was completed on shaken baby syndrome that was facility specific prior to the staff member working alone with children and within seven days of the date of employment or the date of subsidy vendor approval. Evidence: On September 8, 2017, the vendor verified that facility specific training on shaken baby syndrome was obtained by the center staff.

Plan of Correction: The administrator will hold the orientation for shaken baby syndrome.

Standard #: VENDSUB-000-031-B-1
Description: Based on observation and interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an emergency to include evacuation, shelter-in-place and lockdown emergencies. Evidence: During the inspection on September 7, 2017, there were not any reunification plans documented in the emergency preparedness plan as confirmed by the vendor.

Plan of Correction: The administrator will add information to the emergency plan.
