22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted with the director beginning at 11:00 AM and ending at 1:15 PM. There were forty-six children in care, ages 17 months through four years old. The four year old children were observed being read a book and listening to music. The three year old children were observed playing with play dough. The toddlers were observed dancing to music. Five children?s records were reviewed and in compliance. Five staff records were reviewed and in compliance. There were no medications on site. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The emergency escape drill log was observed and in compliance. The most recent shelter-in-place drill was documented as April 6, 2017. The most recent emergency escape drill was documented as May 31, 2017.The most recent fire inspection was documented as January 6, 2017. Standard #: 22VAC40-185-40-E
Description: Based on observation, staff interviews, and documentation review, the center failed to maintain compliance with the terms of the current license. Evidence: On September 11, 2017 there were three children in care that were under the age of two years old. Child A's date of birth was March 4, 2016; child B's date of birth was March 25, 2016; and child C's date of birth was April 27, 2016.The facility is licensed to provide care for children two through ten years old.

Plan of Correction: The owner would submit a request to modify the license to lower the age of children the facility can provide services for.

Description: Based on a review of documentation and staff interview, it was determined that the center did not complete monthly practice evacuation drills. Evidence: The most recent emergency evacuation drill was documented as May 31, 2017.

Plan of Correction: The center will ensure that emergency evacuation drills will be completed and documented on a monthly basis.
