An unannounced Monitoring inspection was conducted in the Center during the morning program on September 7, 2017 and concluded in the afternoon. There were 20 children present; the children were separated in two classrooms. The ages of the children were three and four years old. As reported, 24 children are currently enrolled in the program. The LI observed the children in each classroom. The children were observed during teacher-directed activities and self-selected centers. Staff were observed interacting with the children. A sample of 5 children's records was reviewed. Prescribed medications and applicable documentation was observed for two children. Documentation of the center's annual fire and health inspections was current. The center maintains information for staff on site as required by the licensing standards; the LI reviewed 5 staff records at the time of the monitoring inspection. Complete staff records are retained at the administrative office. A discussion was held with the Lead Teacher and the Compliance Supervisor regarding the vehicles being used by the center to transport children. The LI observed yellow buses transporting the children to and from the center. The licensing standards related to transportation was reviewed with center staff. Standard #: 22VAC40-185-550-H Description: Based on staff report, a document containing local emergency contact information was not available in vehicles that are used to transport children to and from the center. The document shall identify potential shelters, hospitals, evacuation routes, etc. that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.) Evidence: 1. The LI had information that the Center is currently using center-owned buses in addition to a contract service. A document with the required emergency information and identified evacuation routes was not available for review. 2. The center provides transportation to and from the center daily for some of the enrolled children; the center also transports children on planned field trips. Plan of Correction: The Administrative staff will review the center's daily transportation operations. A document will be prepared with the required information and filed in a binder to be taken on the vehicle whenever children are transported. All staff will be informed of the center's transportation binder.
