**Lead teacher requirements **New sworn disclosure statement effective 7/1/2017 ** Out of state CPS checks for staff who have lived outside the state of Virginia in the past 5 years.

Comments:
An unannounced monitoring inspection was conducted with center director and center staff. There were 19 children present, ranging in age from 19 months to five years old. There were 5 staff actively supervising the children during morning activities, lunch time, and preparation for rest. Children ate lunches prepared at home - the center has extra food on site when needed. The Dr. Seuss themed room is nicely decorated. One new staff file was reviewed and is in compliance. Standard #: 22VAC40-185-320-B
Description: Based on observation, the facility failed to have toilet paper accessible to children in each bathroom. Evidence: there is no toilet paper holder in the downstairs girls bathroom. The toilet paper is stored on top of the paper towel dispenser, which is too high for children to reach.

Plan of Correction: Center director will have a toilet paper holder installed in the restroom.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the playground equipment does not have a sufficient area of fall zone to include the child's trajectory in the event of a fall while the equipment is in use. Evidence: the new climber equipment with slide has the following fall zones: the fall zone from the slide to the timber barrier is 37.5", the rock climber side is 64" to the fence, and the ladder is 39" to the fence. The rock climbing "fort" measures 43" to the timber barrier, 44" to the fence, 46" to the back fence, and 35" to the other timber barrier. The wood mulch measured 4 inches in the fall zones.
