An unannounced monitoring inspection was conducted from 8:10am-12:45pm. At the time of entrance, 69 children were in care with 11 staff members present. Children were observed hand washing and snack time, school age children were preparing for school, table toys, engaged in group activities, infants playing with teachers and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The center was clean and organized. All staff had CPR and First Aid training. Standard #: 22VAC40-185-60-A Description: Based on record review, Children's records did not have complete information. Evidence: 1. All six children's records reviewed were missing previous child day care and schools attended by the child. Plan of Correction: I will ask the parents for the information and add it to our tour form.
Standard #: 22VAC40-185-510-N Description: Based on review of medication, when an authorization for medication expires, the center did not dispose of the medication. Evidence: 1. Child A had a medication with an authorization form that expired on 06/30/2017 and another medication with an authorization form that expired on 08/30/2017. Plan of Correction: I will ask the parents to fill out a new updated form.
Standard #: 22VAC40-185-520-C Description: Based on review of diaper ointments and creams, the center did not obtain written authorization from the parent to apply diaper ointment or cream. Evidence: 1. Child B, Child C and Child D had diaper creams labeled with their name on it, but did not have written authorization from the parent. Plan of Correction: I will ask the parents to fill out the form tonight.
