- Technical Assistance:
The licensing inspector reminded the program director to have adequate supplies in the School Age room and update forms for the current center.
An unannounced monitoring inspection was conducted on September 15, 2017, from approximately 11:10AM to 3:15PM. Ten staff were present with thirty-four children. Administrative staff were available to assist. Children were observed preparing for napping and napping. Five children records were reviewed and 12 staff records were reviewed on this date. The exit interview conducted with the program director.
Standard #: 22VAC40-185-70-A Description: Based on 1 of 12 staff records reviewed, the licensee did not ensure each staff record contained all required information. Evidence: During the inspection conducted on September 15, 2017 , the following record for staff #3 had 7 out of 24 hours of documentation of training for program lead. In the record staff was identified as a teacher ( program lead). Plan of Correction: Staff #3 record was changed while inspectors were present with the staff's current position title as a van driver / floor monitor.
Standard #: 22VAC40-185-90--A Description: Based on 1 of 5 records review, the licensee did not ensure there was written agreement between the parent and the center in each child's record by the first day of child's attendance. Evidence: During the inspection conducted on September 15, 2017, child#3 agreements form was signed by parent on May 20, 2016 the center did not open until August 4, 2017. The Center director stated they moved the files from another center that was no longer open and and adjusted the address and start date on the form. The parent did not resigned the parent agreement for the new center. Plan of Correction: The Center director will update the form and have the parent review and sign with correct date.
Standard #: 22VAC40-185-270-A Description: Based on observation, the licensee did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on September 15, 2017, there was one of two buses that had the left mirror black platic frame broken with cracks , sharp edges and pinching points. Plan of Correction: The case over the driver side mirror will be replaced.
Standard #: 22VAC40-185-340-A Description: Based on observation, the licensee did not ensure when staff are supervising children , they shall always ensure their care , protection and guidance their care, protection, and guidance. Evidence: During the inspection conducted on September 15, 2017, child #1 was sitting in a infant seat/ bouncy seat with the straps were not buckled. Plan of Correction: Program Director will retrain staff on the importance of strapping the kids in the bouncy seats.
Standard #: 22VAC40-185-500-B Description: Based on observation, the licensee did not ensure the diapering surface was used only for diapering or cleaning children. Evidence: During the inspection conducted on September 15, 2017 the following was observed in the second 2 year old classroom there were wipes on the diapering surface. Plan of Correction: The program director will have the wipes removed and remind staff that the diapering area is not to be used for storage.
Standard #: 22VAC40-191-60-B Description: Based on 2 of 12 staff records reviewed, the licensee did not ensure staff had a completed sworn statement or affirmation before they are employed or provide volunteer service. Evidence: During the inspection conducted on September 15, 2017, the record for staff #6 (Date of hire 8/29/2017) and staff #9 ( date of hire 9/11/2017) did not have the updated sworn statement or affirmation. Plan of Correction: Program director had it completed when inspectors were present.
